Information you give us
We collect the information you choose to send us, such as:
- your name, email address and message when you use a contact form or email us
- your name and email address when you join our newsletter or respond to an offer on our website
- your phone number and any details you share when you call or text us
Please do not send medical or health information through our website forms or by email. To share health information with us, call us at 425-395-4097 or use our secure online booking system.
Booking, patient records and our online store
Online appointments are booked through Jane (janeapp.com), a secure practice management system. The information you give when booking, completing intake forms or receiving care is part of your patient record. It is protected under federal health privacy law (HIPAA) and our Notice of Privacy Practices, which we provide to every patient. It is not covered by this website policy.
Supplements and products are sold through Fullscript and our online shop. When you use those services, their own privacy policies apply.
Information collected automatically
When you visit our website, some information is collected automatically:
- Analytics. We use Google Analytics to understand how visitors use our website, such as which pages are visited, the type of device and browser used, and the general location (city or region). Children
Our website is not intended for children under 13, and we do not knowingly collect personal information from them online.
